What is chronic care management?

Chronic care management is a service for patients that have multiple chronic health conditions. A chronic health condition is any health problem that lasts 12 or more months. 

MIG internal medicine providers offer chronic care management. MIG has primary care practices that accept all health insurance plans. 

For Medicare patients, chronic care management refers to any non-face-to-face medical care for your long-term health conditions. That includes electronic and telephone communication with your MIG provider, as well as communication with other health professionals. Chronic care management for Medicare patients also includes medication management.

In-office visits are part of chronic care management, but for Medicare, patients' clinic visits are billed separately. 

To provide comprehensive care, your MIG provider uses remote patient monitoring. That allows them to monitor your chronic health conditions and offer care without you having to come to their clinic. The service is especially beneficial to Medicare patients.

Your MIG provider determines if you benefit from chronic care management. If you have two or more chronic health conditions, then your MIG provider might consider you a candidate for chronic care management.
